Kung fu, Sanda and related training are physically demanding activities that carry an inherent risk of injury. By signing below I confirm that I am participating voluntarily and that I am in adequate physical condition to train.
I accept responsibility for my own health and safety during training, understand that I should stop and seek help if I feel unwell or injured, and agree to follow the instructions of the masters and staff at Yu Kung Fu Academy at all times.
I release Yu Kung Fu Academy, its masters, staff and the host temple from liability for injury, loss or damage arising from my participation, except where caused by gross negligence. I confirm I hold, or will arrange, my own travel and medical insurance for the duration of my stay.
Fees are non-refundable. We keep the school small and hold a place for every student who commits, so once you register your registration fee and tuition are not returned. Kung fu also asks a lot of you — early mornings, long days, and real effort. If, deep down, you know you would give up easily, this may not be the school for you, and that is completely okay. But if you are ready to stay the course, we will give you everything we have.
If the applicant is under 18 years of age, a parent or legal guardian must read and sign this waiver on their behalf.
